Data Protection Policy

The The National Kidney Foundation (NKF) collects personal data (PD) from individuals for its operational needs as it fulfills its role as a charitable organisation that provides dialysis and rehabilitative care to kidney failure patients.

  1. Personal Data
    In this Data Protection Policy, “Personal Data” refers to any data or information about an individual from which one can be identified either (a) from that data; or (b) from that data and other information to which we have or are likely to have access to. Examples of such PD include:
    • Name, NRIC, passport or other identification number, telephone number(s), mailing address, email address and any other information relating to an individual provided to us in any form;
    • Information about one’s use of NKF’s website and services and medical history information;
    • Employment history, education background and income levels; and
    • Financial transaction-related information, such as bank account or credit card information or donation history.
  2. Policies and Procedures
    Adequate policies and procedures have been put in place by NKF to:
    • Specify the purpose(s) for use of PD.
    • Safeguard the interests of the individuals and protect the confidentiality of the PD that it has collected.
    • Retain the PD for only as long as it needs to meet its purpose.

    NKF seeks implied consent from individuals for the collection, use and disclosure of PD to:

    • Render an NKF service that the individual has applied for.
    • Follow-up on queries or requests in NKF’s Feedback Form or Online Patient / General Enquiries Form submitted by the individual.
  3. Use and Disclosure of Personal Data
    NKF shares only necessary PD with other Government agencies or non-Government organisations authorised by NKF to carry out specific functions in order to:
    • Process any applications made by the individual.
    • Render an efficient and effective service to the individual, unless the sharing of the PD is prohibited by legislation.

    NKF will ensure that the said Parties satisfy NKF’s Data Protection Policy requirements before the release of the PD.

  4. Withdrawal of Consent, Access, Correction of Personal Data
    Individuals may contact NKF via NKF’s Online Patient / General Enquiries Form or contact NKF’s Manager (Operations and Strategic Planning) to:
    1. Request for amendments to his / her Data on the basis that the PD collected or maintained by NKF is inaccurate or incomplete.
    2. Provide feedback in relation to NKF’s Data Protection Policy.
    3. Withdraw Consent to any use or disclosure of PD.

    NKF will ensure that the said Parties satisfy NKF’s Data Protection Policy requirements before the release of the PD.
